Moving Pharmacy Forward: Using the Pharmacists’ Patient Care Process

Over the past few years, the idea of pharmacist provider status and further development and expansion of the roles of pharmacists has been at the forefront of the profession of pharmacy. To meet the needs of the always changing health care system and to standardize patient care, the Joint Commission of Pharmacy Practitioners (JCPP) began an initiative to develop a process by which pharmacists could approach patient care. Between 2012 and 2014, JCPP created a working group consisting of representation from numerous national pharmacy organizations to develop a consistent patient care process to be used by pharmacists. Their vision was to create a process by which pharmacists could play a vital role in achieving optimal health and medications outcomes for patients. This process became known as the Pharmacists’ Patient Care Process (PPCP). Attendees of this session will be introduced to the PPCP elements, and strategies on how to implement this process will be discussed. The PPCP will then be applied to a patient case with the goal of enhancing patient outcomes.

At the completion of this activity, participants will be able to:

  1. List the five elements of the Pharmacists’ Patient Care Process.
  2. Discuss strategies to incorporate pharmacy technicians in the Pharmacists’ Patient Care Process.
  3. Determine how to use the Pharmacists’ Patient Care Process to optimize patient health and medication outcomes.
  4. Apply the Pharmacists’ Patient Care Process to a patient case.

Technician Learning Objectives

At the completion of this activity, participants will be able to:
