Charlene McCormack, PharmD, BCPS is the Clinical Coordinator and Soldier Centered Medical Homes Pharmacist. She manages the sole provider program and is the institutional subject matter expert for CHCS and Careport report analysis. Dr. McCormack is the primary clinical pharmacist for the medication review for all deploying rotation units of the Big Red One, 1ID. She is the cornerstone of the success of the opioid prescribing reductions at Irwin Army Community Hospital.
